What do I do if I forget my password?

To reset your password:
  1. Click the Forgot Password link on the top of a Provider Portal page. The Forgot Password page is displayed.
  2. Enter the e-mail address that you used to create your account in the Email Address text box.
  3. Click the Submit button. An e-mail is sent to your e-mail address that contains a link to reset your password.
  4. Click the link in the e-mail. The Reset Password page is displayed.
  5. Enter a new password in the New Password and Repeat New Password text boxes.
  6. Click the Submit button. The Password Recovered page is displayed and your new password is saved.
