To send us feedback about the Provider Portal Web site:
- Click the Feedback link. The Site Feedback window is displayed.
- Select the type from the Feedback type drop-down list. The following options are available:
- Broken Link
- General Feedback
- Website Issues
- If necessary, change the text in the Name and Email address text boxes
- Enter a subject in the Subject text box.
- Enter the feedback in the Comment text box.
- Click the Submit Feedback button.
