Can I deactivate my account?

To deactivate your account:
  1. Log on to Provider Portal.
  2. Click the Account Settings link at the top of a page. The Edit Profile page is displayed.
  3. Click the Deactivate Account link.
  4. Enter your password in the Account Password text box.
  5. Click Continue.
  6. The Confirm Account Deactivation page is displayed.
  7. Click the Deactivate My Account button. Your account is deactivated and the Login page is displayed.
